Clinical Interest

ACTH (adrenocorticotropic hormone) measurement is indicated in the evaluation of adrenal and pituitary disorders.

In the diagnosis of Cushing's syndrome, it is used to differentiate between ACTH-dependent causes (e.g. pituitary adenoma, ectopic ACTH production) and ACTH-independent causes (e.g. adrenal tumours).

  • In Addison's disease, low cortisol levels with high ACTH indicate primary adrenal insufficiency.
  • In hypopituitarism, low ACTH with low cortisol suggests secondary adrenal insufficiency due to pituitary dysfunction.
  • It helps to differentiate the causes of hypercortisolism by determining whether the excess cortisol is due to pituitary or ectopic secretion of ACTH.
